Please find attached the short, multiple choice Participant Questionnaire (PQ) for Non Schema Release 2014 – MCR 1137.

All Participants who have been sent the PQ are required to complete and return to RMDS by the 14 March 2014.

The purpose behind the PQ is to ensure that you:
• Are aware of MCR 1137
• Understand the detail of the changes being made
• Understand how the change will (might) affect you
• Are prepared for the change.

The PQ is multiple choice based and comprises of 7 short questions. We don’t envisage it taking more than a few minutes to complete.
